Case Manager

Rachel Chmura is a dedicated Case Manager who is passionate about helping individuals navigate the recovery process with compassion, encouragement, and personalized support. Since joining United Recovery Project in 2020, Rachel has demonstrated exceptional commitment to patient care, beginning her career as a Behavioral Health Technician before advancing into her current role as Case Manager.

Originally from New London, Connecticut, Rachel relocated to South Florida in 2018, where she began her own recovery journey. Her lived experience provides a unique perspective that allows her to connect with patients and families with genuine empathy, understanding, and hope. She is committed to helping each individual overcome barriers to recovery while ensuring they have the resources and support needed to achieve long-term success.

As a Case Manager, Rachel works closely with patients, families, and the clinical team to coordinate treatment services, develop individualized discharge and aftercare plans, and connect clients with community resources that support continued recovery. She is dedicated to advocating for every patient while helping them build a strong foundation for life after treatment.

Rachel believes that recovery is possible for everyone and finds great fulfillment in supporting individuals as they create healthier, more meaningful lives. Her compassion, dedication, and commitment to serving others make her a valued member of the United Recovery Project team.
