What Are the ASAM Levels of Care?
Before we look at the levels of care, it’s helpful to get a definition of ASAM.
ASAM stands for the American Society of Addiction Medicine. It is the leading professional organization of addiction specialists, and this group of experts is responsible for developing a framework for connecting individuals with the appropriate level of care based on their needs.
It’s helpful to think of the ASAM levels as a “roadmap” for recovery. Just like you’d program a GPS to find the various different routes based on traffic, ASAM criteria can help determine which level of support may best help someone based on their level of addiction and particular situation.
The levels exist to ensure that individuals receive care that is neither too intensive (which can be unnecessary and costly) nor too minimal (which might not provide enough support).
Modern professional treatment centers and healthcare providers (as well as many insurance companies) use the ASAM levels as the criteria for determining appropriate addiction care.

What Are The Different ASAM Levels?
The ASAM levels range from early intervention methods to intensive, inpatient treatment options. Each level addresses different needs for individuals and can provide varying levels of structure at each step of the recovery process.
Level 0.5 — Early Intervention Services
Early intervention is the first ASAM level. Level 0.5 isn’t full treatment, but instead shows a targeted approach for those who might be at risk of developing substance use disorders.
At this level, you’ll find screening, education, and counseling for those who may show warning signs of addiction. It’s also a starting place for those who have been referred due to circumstances like DUI charges.
At level 0.5, the primary goal is prevention and early recognition before addiction develops further.
Level 1 — Outpatient Services
Level one is where standard outpatient treatment comes into play.
These include therapy sessions that last for less than nine hours weekly, which allow participants to stay engaged in their normal routines.
Level 1 is often a starting spot for those with mild addiction, and also works well as a step-down level after more intensive treatment. At this level, treatment often focuses on relapse prevention and addressing underlying issues.
Level 2 — Intensive Outpatient/Partial Hospitalization
Level 2 services provide more structure while still allowing people to live at home. This level is typically split into two primary categories:
- Level 2.1 (Intensive Outpatient or IOP): Level 2.1 involves 9-19 hours of treatment weekly, and is usually scheduled around work or school commitments.
- Level 2.5 (Partial Hospitalization or PHP): 2.2 provides 20+ hours of treatment per week, and often occurs five days a week.
These programs often serve as a bridge for those who may not be ready for residential treatment or to support them as they move back into daily life following Level 3 care.
Level 3 — Residential/Inpatient Treatment
When individuals need more substantive care, Level 3 may be necessary. This level includes different forms of 24-hour care in non-hospital settings:
- Level 3.1 involves a low-intensity residential treatment with at least five hours of clinical services weekly.
- Level 3.3 provides medium-intensity care for those with cognitive or functional impairments, such as developmental challenges or traumatic brain injuries.
- Level 3.5 includes high-intensity residential treatment for those struggling from substance use disorders and/or co-occurring mental health conditions.
- Level 3.7 offers medically monitored intensive inpatient treatment with 24-hour nursing care and physician availability for patients with complex needs.
Level 4 — Medically Managed Intensive Inpatient
Level 4 is the highest level of care, and involves 24/7 supervision in a hospital setting. This is an appropriate level for those who may be experiencing severe addiction withdrawal or medical complications due to their addiction.
